Notarized Affidavit-B  

4.

Employment proof ( Drug licenses/Teaching/Industry etc ID proof )

5.

Residential proof(Election ID/Aadhar card/Bank account/Indian Passport/ration card/driving license)

6.

Photocopy of RP Certificate

7.

Passport size 4 colour photos

8.

Fees (Cash/DD)

9.

Collect Duplicate Pass Book on same day

III.

Fill the application and tag all the documents in sequence as per CHECKLIST

IV.

Submit application IN PERSON in the office of Andhra Pradesh Pharmacy Council.

V.

Get Originals back after verification.
